Physical Exams: Students in Grades 4, 7, and 10

MA Department of Public Health regulations require that every student provide a written copy of a physical exam periodically to the school nurse. Canton Public Schools policy is that students in grades 4, 7, and 10 will submit a new physical upon entry into these grades.

Students who do not have a physical exam that documents the required age-appropriate immunizations will be subject to exclusion from school until documentation of immunizations is provided.
