Important Forms and Files
The Canton
Public Schools are in the process of going "green" with all of
our forms. We are working to have all forms on line so
that the public can easily access them. We are also
hoping that this will help make sure that forms are sent to the
correct recipient.
To complete
the forms, please follow the directions below.

Click on the Adobe Reader button to download the
latest version ( 9) of Adobe Reader (this will allow
you to view, edit and save the form)

3. Fill in the fields with the appropriate information |

Click the
"SAVE" button to save the form |

Email the form to the appropriate person (i.e.
