The four Required Forms are to
be completed and passed into your coach or mailed
prior to the season to: Danny Erickson, Canton High School, 900 Washington St.,
Canton, MA 02021.

MIAA
Medical Information form and Physical Exam form
All student-athletes and parents must fill out the MIAA Medical Information
form which is the FIRST PAGE of the above link.
You may choose to use the Physical Form supplied here (the second page) or use one from your
physician.

Department Rules and Regulations Agreement Form
To be signed by student-athlete and parent after
reading Rules and Regulations
YOU MUST CLICK HERE and read the attached
document, before signing this form.
Required Form #3
Medical Waiver Form
Form allows student-athlete to be treated by a
physician if needed.
Required Form #4
Liability Form
Deals with Town's liability
User Fee Form
Form is to be filled out and check sent to CHS
Athletic Department by deadlines.
